1095-C Tax Form Information - Current & Former Employees

How to Obtain Your 1095-C Tax Form

Employees who were eligible for or enrolled in company medical coverage may receive a Form 1095-C for your tax records.

Forms are typically available by March 1.

You do not need this form to file your taxes, but keep it for your records.

You may receive a 1095-C if:

You were a full-time employee during the year, or
You were enrolled in the company’s medical plan at any time during the year.

If you were not offered coverage or were not enrolled, you may not receive a form.
